The following blood chemistry data is needed for the KAMM-12 report:

1. Albumin

2. Calcium

3. Phosphorous

4. SGOT (AST)

5. Glucose

6. Alkaline Phosphotase

7. LDH

8. Total Bilirubin

9. BUN (Urea Nitrogen)

10. Uric Acid

11. Cholesterol

12. Total Protein

Also, the following information: Customer's name, age, sex, lab visit date, and lab report date, will be required as part of the data collection process.

If the data is not available, and the distributor / customer cannot find a facility to do the necessary lab work, the distributor can order the lab work as part of the KAMM-12 order.
