We are looking for distributors!
Personal Health Response (PHR) is in the business of developing health / wellness
reporting products and services. We rely on distributors to promote and sell the
KAMM-12.
The KAMM-12 can be marketed as a stand-alone product, or in conjunction with
health / wellness related business activities such as weight management,
nutritional, or other fitness
programs.
If you are in the business of providing health / wellness products or
services, we invite you to become a PHR distributor. The process is easy:

request the Distributor Application Form. |
| We will send you the Distributor Application Form. Please complete the forms and return them to PHR. |

Wait for notice of approval, which should be with 7 to 10 days. If you were
approved, we'll send you the approval notice and 2 copies of the Distributor
Agreement. Sign and return both copies for our endorsement. One of
the endorsed originals will be returned for your records. |
| Once your Distributor Agreement is in place, you will be authorized to access our distributor resources.
Access will require the username and temporary password assigned in the
approval correspondence. You will change the password on first use. |
| Note: part of becoming a distributor is that you order two KAMM-12 reports: one within 30 days and the second report within 120 days. We
require the two reports because we want our distributors to become familiar
with the product and realize the value of it. |
